MediCal in California covers everyone who are at the bottom end of the poverty scale, who cannot afford to pay anything in Obamacare. For example, all homeless people in the state are covered by this no fee healthcare system.
What does Medi-Cal cover?

The difference between Medi-Cal and MediCare is that Medi-Cal has no payment at all attached to it monthly, but it is not accepted by many doctors, healthcare providers or hospitals.
MediCARE is accepted by many more hospitals and doctors plus healthcare providers, but there is a cost associated with it monthly, which is a minimum of $100 or so per month. For someone who makes only $400 per month, this is not affordable.

AMHERST — Presidential candidate Bernie Sanders’ “Medicare for all” plan, unveiled Sunday hours before the televised Democratic debate in South Carolina, got a seal of approval from University of Massachusetts Economics Professor Gerald Friedman, who has argued for such a plan for decades.
Friedman wrote a 2013 paper for the non-profit organization Physicians for a National Health Program stating that such a plan would both save money and lead to higher-quality care.
Familiar with this work, Sanders’ campaign asked Friedman to run numbers on the candidate’s plan. The result was that Friedman found that a family of four making $50,000 in annual wages would pay $466 more in taxes but would save $4,955 annually from elimination of health insurance premiums and $1,318 in deductibles, making for a total savings of $5,807 under Sanders’ proposal.

Most states require nursing-home residents to spend virtually all of their assets -- down to as little as $2,000 -- before they may qualify. Married couples have higher asset allowances as long as one spouse is healthy enough to remain at home.
A year in a nursing home costs about $75,000 nationwide (substantially more in the Northeast and California), so it's easy to wipe out a lifetime of savings. You can sidestep the Medicaid issue by buying long-term-care insurance if you or your parents are healthy enough to qualify for coverage and can afford the premiums. A policy that provides coverage for five years of care at $143 per day with inflation protection costs about $2,000 a year or more, depending on your age.
For some individuals, long-term-care insurance may not be an option. And some people give away their money and property in order to qualify for Medicaid help sooner, a practice known as Medicaid planning. One way to deal with both situations is to earmark funds sufficient to pay for care, then establish an irrevocable trust to remove remaining assets from the estate, says Barbara Culver, president of Resonate, a wealth advisory service in Cincinnati.
